Dear Athlete,

Congratulations on choosing RTA to design and oversee your training program! 

The Athlete Questionnaire is one of the most important forms you will share with your coach, and will allow thorough development of a personalized training program.

Plan on taking ~20-30 minutes to complete the form, and do not hesitate to contact your coach if there is additional information you wish to share.

    This section is VERY IMPORTANT as it will allow your coach to create a schedule that fits your life as best as possible.

    Use the space below to let us know:

    What is your typical daily availability to train EACH day? (Monday - Sunday; Hours / Minutes)

    Are there any days you CANNOT swim, bike or run due to your
    typical schedule (i.e. the pool hours prohibit me from swimming on Sundays).

    Do you have a preference for your rest day(s)?
    How much time do you have available to train each day? Are there any restrictions we should know about?
    If not, that's fine too.
    Example:

    ​Mon. - 1 hr (45 min max for swim)
    Tues. - 1.5 hrs
    Wed. - 1 hr (cannot swim)
    Thurs. - 1.5 
    Fri. - 2 hrs
    Sat. - 3 hrs
    Sun. - 2 hrs

    Monday's would be good to have off if possible
